Each student is responsible for completing a number of forms and provide copies of important documents in order to attend the trip.

(only required if they apply to your child)

Form 4 Medical Authorization: Required for ANY prescribed medication the student needs to take during the Disney Trip.

Form 5 Asthma: Required for any student who has Asthma
​
Form 6 Epinephrine Auto Injector: Required for any student who has allergies and is required to carry an Epinephrine Auto Injector
